Reservation Agreement for use of Good Works’ Recreational Facilities

Thank you for using Good Works’ Recreational Facilities for your group gathering. We want to make this available to you and hope that your time with us is a positive one. Your donation will assist one of our residents to earn some money and provide them with work experience. This form will help us to prepare for your event.

Requested donation: $75.00 for a morning or afternoon event. $100.00 for an all-day event. A $35 non-refundable deposit is due with your reservation. The balance will be due the day of the event. If weather prevents you from holding your event, you may reschedule it. If that is not possible, we will refund your deposit.

Parking is available in designated gravel areas only. Any damage done to the property by vehicles parking in undesignated areas or on the grass, will be the responsibility of the group to repair. We ask that before you leave from your activity that you clean up the areas you used. Your host is available to you for whatever you need for the entire time you are here. Please feel free to request from him/her any assistance you may need.

We agree to honor the following guidelines:

1. We will use only designated areas for smoking (side porch) and extinguish cigarettes in can provided, not throwing them on the ground.
2. We agree to clean up all trash in the areas we use, leaving them in good condition.
3. We agree to not hold Good Works responsible for items left behind by the group. …
4. We will not hold Good Works responsible for any injuries that occur.
5. We agree to use the recreational facilities as is and at our own risk.
